FORM INSTRUCTIONS:
- You’ll want to save the form to your computer FIRST (right click the button below, and click Save As or Save Target As to save it directly).
- Then open it in Adobe Reader (it is a PDF document). If you don’t have that program, you can download it here.
- Please TYPE your answers directly into the form. Checkboxes can be selected easily as well with a mouse click.
- Then save the document back to your computer.
- That should allow you to then email it to me as an attachment.
